In response to a number of questions about speech therapy for children with LKS the following guidelines are provided, from a UK perspective: (I) There are no published efficacy studies of speech and language therapy for children with LKS. There is one longitudinal follow-up study of children who attended a residential school in 1970-80's (Ripley and Lea 1984) and one single case study of a younger child in a residential school (Vance 1991). (II) All other discussions about speech and language therapy with children with LKS are based on unpublished clinical work. (III) My personal view about speech and language therapy for children with LKS is based on 15 years of my own clinical and research work with these children. The provision of education and health care in the UK at this particular time, and the history of the development of these services, has its own contribution to make to the was in which questions about speech and language therapy for children with LKS are both asked and answered. Specialist residential schools do often have an integral speech therapy service, but most ordinary day schools will rely on a speech therapy service provided to them by the local health service. Despite the fact that children are said to have rights under the Children Act and the Education Act the question of who is responsible for providing speech and language therapy as recommended for any particular child is still too often a political football. | |||

I even called them at there 1-800 calljeff number. They spoke in depth to me and referred me to another phone # . She has avg.non verbal intelligence,artisic like behavior, and appeared totally deaf(which was ruled out by hearing test) She is under a neuro's care. All children with LKS have abnormal electrical activity in one, sometimes both temporal lobes, the area of the brain responsible among other functions for processing language. Some children never have obvious seizures: it is not necessary to have a seizure to have LKS. A few children have many seizures which are harder to control. Some children have episodes of very abnormal "autistic type behaviour" with symptoms such as avoidance of contact with family and friends (avoidance of eye contact is common) extreme pickiness over food, very disturbed sleep, attacks of rage and aggression, insensitivity to pain, bizarre and inappropriate and repetitive play. Some children may recover their language fully, but many are left with a language disability. Children with little or no language comprehension should be introduced to a sign language as early as possible. Education should be in a special class or school for children with severe speech and language disorders (or if this is not possible occasionally, in a school for the deaf). | |||

Epilepsy is a chronic disorder of the brain characterized by recurrent seizures, as opposed to seizures occurring in association with high fever, drug effects, chemical imbalance (e.g., low blood sugar). If an autistic person's suspected "seizures" are clearly the consequence of anger, frustration, fear, these episodes are probably not seizures. Seizures can also be brought on by prolonged hyperventilation in a person susceptible to seizures. Seizures generally follow a similar -pattern each time, although some seizures might be more intense and prolonged than others. If the autistic person's "seizures" are varied in movements and mannerisms, these events are probably not seizures. The seizures and the aphasia may both reflect abnormal brain functioning (due to inflammation, growths, unknown structural or neurochemical abnormalities) or the aphasia may be a consequence of the seizure discharges. | |||

Acquired epiletiform aphasia (AEA) or the Landau-Kleffner Syndrome is the lost of language associated with an abnormal Electroencephalogram (EEG). Typically, the onset of this type of language difficulty occurs between the third and seventh years of life in a child who had been developing normally. The EEG abnormalities (paroxysms) include bilateral spikes in the temporal or temporal and parietal lobes of the brain (at some point in the illness, all will exhibit this, especially during slow sleep); bilateral slow waves which are maximal over the temporal lobes; unifocal, multifocal or generalized spikes or spikes and slow waves; and continuous slow waves during slow sleep. The function of the motor part of the brain is much less complex and there is more redundancy in brain tissue to perform this task than is the case with the regions of the brain involved in language. Consequently, it would not be surprising to see that the language centers of the brain in a young child would be particularily sensitive to seizure activity, especially if it were to occur during periods critical to language development. Benign partial epilepsy of childhood, AEA and continuous spike-waves during slow sleep (CSWS) are three enities with identical EEG paroxysms recorded from different regions of the brain. | |||

Landau-Kleffner Syndrome Landau-Kleffner Syndrome and Autistic Regression are mystifying conditions in children that are poorly understood by the medical community. We have been using a multidisclipinary approach to evaluate and diagnose children who are suspected of having these conditions. The ability to produce and understand language is the most complex task the human brain performs and it is remarkable that in most children this ability evolves without difficulty. In some children, like those with Landau-Kleffner syndrome, something affects the way the brain functions or develops, and this causes language development to be disturbed. This may not necessarily occur at the same time as the regression in language.About 30 percent of LKS children do not exhibit obvious signs of seizures and identification of the brain electrical problem may require specialized medical testing such as an electroencephalogram (EEG) to be observed. These tests look at brain function in different ways, and may be needed to reveal the abnormalities associated with LKS. Behavioral disturbances are apparent in about 2/3 of LKS cases, and range from aggressivity and attention deficit hyperactivity disorder symptomology, to behaviors commonly associated with autism such as avoidant and withdrawn behavior, resistance to change in daily activity, gestural stereotypies, echolalia and echopraxia, hyperlexia and even psychotic-like disturbances. | |||

June 5, 1997 (15:52) I am the parent of a 6 year old child who up until August of 1996 had expressive speech. My question is how can FFW help a child such as mine who has such limited expressive language? During the beta test of FFW, I understand that out of a test group of 500 kids approximately 4 or 5 of them were children with Landau-Kleffner syndrome. One of the parents of a child who beta tested FFW is part of the LKS Parent Support Group I communicate w/ via e-mail. My questions are: In the beta test group were there any Landau-Kleffner children with a severe form of apraxia such as that of my daughter -- having absolutely no expressive speech? Granted, each child is different and Landau-Kleffner is usually accompanied with a host of neurological and behavioral problems, however, I would be very interested in finding the answers to my questions, as I am considering having my child enrolled in Fast ForWord this summer. | |||

As people age, and as epilepsy, maybe, reduces the brain power available, so our ability to remeber things seems to decrease. Would like to share with parents of children w/hyperlexia interventions--largely nonpharmaceutical and definitely nonelectroshock-that worked for me, and address issues that may arise in the teen and adult years. After finding the AHA website we took Alan to the Center for Speech and Language in Elmhurst, Illinois where his hyperlexia was confirmed. Our son, John (11-11-91), was diagnosed shortly before his 3rd birthday with Developmental Apraxia of Speech, Hyperlexia and Sensory Integration Disorder. Valparaiso, Indiana--We are Ed and Pam Byrne (e-mail: ebyrne@exodus.valpo.edu ), parents of a four-year-old son (born 12-16-91), who has just been diagnosed as hyperlexic by our local school district, where is is now enrolled in a special preschool and is receiving speech therapy. Since Alex is an only child, we are hopeful that the preschool will help improve his social skills as well as his oral language skills. | |||

Landau-Kleffner syndrome (acquired epileptiform aphasia) is a disorder in which young children with previously normal language development undergo a deterioration in language skills in association with epileptiform (seizure-like) abnormalities on EEG. Between 70 - 80% of children with acquired epileptic aphasia have seizures, but these are not mandatory for diagnosis. Some of the children may have associated behavioral abnormalities including hyperactivity, decreased attention, distractibility, temper tantrums, aggressiveness and social withdrawal. The cause of this disorder is not known, but the EEG epileptiform abnormalities may be in part responsible for the dysfunction. A small number of children have been treated with steroids with encouraging improvement in their language. At the AECOM/Montefiore Comprehensive Epilepsy Management Center, we are treating eligible children with language regression, using a multidisciplinary approach. | |||

Laundau-Kleffner Syndrome Speech Evaluation in Laundau-Kleffner Syndrome Nonverbal Testing in Landau-Kleffner Sydrome Questions & Answers Meet the Director List of Staff and Important Phone Numbers This edition of the Rush Epilepsy Center Newsletter is devoted to Landau-Kleffner Syndro me (LKS). We are featuring this pediatric syndrome because Rush has treated many of the se children and has offered a surgical treatment that, until recently, has not been offe red at other institutions. Those children who have been exposed to sign language in the initial stages of the disease retain the sign language and will rely on this modality to express themselves until the expressive language develops. LKS is an acquired epileptic aphasia that is associated with seizure activity, severe electroencephalogram (EEG) abnormality, and loss of language in children whose development is otherwise normal. While the initial report suggested that most children had spontaneous recovery of speech, more recent clinical studies suggest that many children do not recover speech though their EEG abnormality improves in adolescence. The amount of speech that is lost can vary from child to child, but many children are left with little language ability. | |||

Landau-Kleffner syndrome (LKS) is a childhood disorder. A major feature of LKS is the gradual or sudden loss of the ability to understand and use spoken language. All children with LKS have abnormal electrical brain waves that can be documented by an electroencephalogram (EEG), a recording of the electric activity of the brain. Approximately 80 percent of the children with LKS have one or more epileptic seizures that usually occur at night. There have been no reports of children who have a family history of LKS. Therefore, LKS is not likely to be an inherited disorder. There have not been many long-term follow-up studies of children with LKS. This lack of evidence, along with the wide range of differences among affected children, makes it impossible to predict the outcome of this disorder. | |||

It is twice as common in males than females. Initially, these individuals have a healthy, problem-free development with normal speech and vocabulary. These individuals first lose their ability to comprehend (i.e., receptive speech) and then their ability to speak (i.e., expressive speech). One common characteristic of Landau-Kleffner Syndrome, which is often diagnosed in conjunction with autism, is the failure to respond to sounds. Autistic characteristics seen in Landau-Kleffner Syndrome individuals include pain insensitivity, aggression, poor eye contact, insistence on sameness, and sleep problems. The prognosis is better when the onset is after age 6 and when speech therapy is started early. | |||

We are the UK support group and national charity for children with Landau-Kleffner Syndrome, their families and interested professionals. Over the last year it has become clear that the number of families of children with LKS or related conditions in the UK has risen to mean that a national group centred around these children's needs is now possible. Janet Lees: is a speech and language therapist who does clinical and research work with children with LKS and is available for speech therapist advise, and information 'on-line' via the LKS web site. Most parents in UK probably do not have on-line facilities, and we are surprised by the number of US enquiries - we're just sifting through some of what we've been told about what's available in the U.S.. These individuals are often referred to as having 'regressive' autism. Since there are no physiological tests at this time to determine whether a person has autism, the diagnosis of autism is given when an individual displays a number of characteristic behaviors. These individuals are often thought of as having 'regressive' autism because they appear to be normal until sometime between ages 3 and 7. Although there is no known unique cause of autism, there is growing evidence that autism can be caused by a variety of problems. As a result, it may be difficult for individuals with autism to process incoming sensory information properly. Although some individuals are given medications to improve general well-being, there is no primary drug which has been shown to be consistently effective in treating symptoms of autism. | |||

A child with CDD is often indistinguishable from a child with autism. Although some children show autistic patterns of social interaction almost from birth, it is difficult to give a definitive diagnosis of autism before age three. There is no medication that can cure autism, and no one medication that is recommended for individuals with autism. If I have one child with autism, what are my chances of having another child with autism? Current data suggest that the likelihood of having a child with autism if the biological parents already have one child with autism is at least 1/20. This rate may be an underestimate, given that many families with one autistic child will stop having children due to stress or the fear of having another child with the disorder. | |||
